CHELAN ATHLETIC BOOSTER CLUB

PROPOSAL REQUEST

Requests of any nature to the Chelan Athletic Booster Club (CABC) should be made in writing using this form. Proposal Requests should be received prior to the need or event occurring, with time allowed to address the request. Each proposal will be addressed at the next scheduled CABC meeting.

Requests may be made in person in conjunction with this form, or in writing only. It is the responsibility of the person making the request to complete this form. Third party proposals will be redirected to the originator. This is to allow for clear understanding of the request.

Forms may be returned to any CABC director or the high school athletic director.
