Counseling Forms

While occasional and/or brief counseling takes place throughout the school on a daily basis in the form of conflict resolution, crisis management, brief solutions, parent and teacher contact, and classroom guidance, should your child require more intense, regular, planned, or prolonged counseling throughout the school year, parents and guardians must submit a consent to counsel form. If you would like to refer your child for any form (individual or group) of counseling or would like an outside referral arranged for you, please call the counselor at 682-4031 to discuss the issue and also complete the form below and return to the school. Thank you.
